The City invites residents to participate in a short survey to identify and prioritize the most pressing health needs within the community.

The City of Allentown has launched the Health Bureau's inaugural Community Health Needs Assessment (CHNA), inviting residents to participate in a short survey to identify and prioritize the most pressing health needs within the community. 

The insights gathered from this survey will serve as the foundation for the development and implementation of programs designed to benefit the community and elevate the overall health and well-being of city residents.

Residents can access the survey at www.allentownpa.gov/CHNA. Completing the survey typically takes between 5-10 minutes. The survey is also available in Spanish by selecting the "Default language" link at the top and switching to Spanish.

The Allentown Health Bureau is also actively seeking to engage community partners and groups to facilitate in-person surveys and broaden distribution. If you or your organization would like a representative from the Allentown Health Bureau to attend your event and promote the survey, please reach out to Yvonne Kingon at 610-437-7760 ext. 2842 or via email at yvonne.kingon@allentownpa.gov.

For those who prefer paper copies of the survey, they will be available during outreach events. Please contact Yvonne Kingon at the Allentown Health Bureau to request a paper copy of the survey or visit the Allentown Health Bureau at 245 N. 6th Street.

The survey results will be available in 2024 and will be used to help create the Community Health Improvement Plan for the City of Allentown.

La ciudad de Allentown lanzó la Evaluación de Necesidades de Salud Comunitaria (CHNA) inaugural de la Oficina de Salud, invitando a los residentes a participar en una breve encuesta para identificar y priorizar las necesidades de salud más urgentes dentro de la comunidad.

Los conocimientos recopilados en esta encuesta servirán como base para el desarrollo y la implementación de programas diseñados para beneficiar a la comunidad y elevar la salud y el bienestar general de los residentes de la ciudad. Los residentes pueden acceder a la encuesta en www.allentownpa.gov/CHNA

Completar la encuesta suele tardar entre 5 y 10 minutos. La encuesta también está disponible en español seleccionando el enlace "Idioma predeterminado" en la parte superior y cambiando a español. La Oficina de Salud de Allentown también busca activamente involucrar a socios y grupos comunitarios para facilitar encuestas en persona y ampliar la distribución.
