Program Description:
The Nurse-Family Partnership is a comprehensive program in which nurses visit womenand families in their homes and link them with the services they need during pregnancy and for the first two years of the child’s life

Some of the program goals are:

1) To improve pregnancy outcomes

2) Promote child health and development

3) Promote maternal life course development

Home visitation schedule:

• During pregnancy Every week for the first 4 weeks. Then every other week for the rest of the pregnancy
• After the baby is born Once a week for 6 weeks. Then every other week until the baby is 21 months. Then once a month until the baby’s 2nd birthday
